MetaStar launches effort to bring Practice Transformation Network funding to Wisconsin

Hospital, health system, clinic, and physician support needed by Dec 15 to secure funding

Madison, Wisconsin — MetaStar has launched an effort to secure funding for Wisconsin providers to transition their practices and thrive in the pay-for-value environment. If successful, MetaStar will provide no-cost support to primary care and specialty clinicians who want to use their health information technology to improve efficiency in their practice and to provide better coordination of care for their patients.

The Center for Medicare and Medicaid Innovations (CMMI) recently announced the Transforming Clinical Practice Initiative, which will support 150,000 clinicians nationwide to sustain practice and care improvements that will improve health outcomes for patients, reduce unnecessary hospitalizations, save $1 to $4 billion health care dollars nationwide, and ready providers to participate in incentive programs and practice models that reward value.

MetaStar is part of a consortium of organizations across the country that will be collaborating to secure funding for PTN assistance for their respective regions. This consortium includes five of the most successful Regional Extension Centers (RECs) and Quality Innovation Network Quality Improvement Organizations (QIN-QIOs), along with their Regional Health Improvement Collaboratives (RHICs). If funding is awarded to the consortium, MetaStar will partner with the Wisconsin Hospital Association, Wisconsin Collaborative for Healthcare Quality, the Wisconsin Medical Society, and the Wisconsin Primary Health Care Association as well as other stakeholders to ensure harmonization of efforts and avoid duplication in Wisconsin.

To demonstrate support for this effort to secure CMMI funding in Wisconsin, MetaStar is asking hospitals, health systems, clinics and physicians to complete an Intent to Participate form found at www.metastar.com/ptn by December 15. Submitting an Intent to Participate form ensures contact from MetaStar regarding this opportunity if funding is awarded.
