MercyCare Recognized for Excellence in Diabetes Management

JANESVILLE, WI—MercyCare Health Plans’ diabetes case management program was nationally recognized by earning a 2012 Case In Point Platinum Award. MercyCare is honored for demonstrating leadership, value and commitment toward continuous improvement in diabetes management.

“Being chosen as a recipient of this award confirms our diabetes case management program is the best of the best,” said Dean Wolanyk, MD. “It encourages our hope that our work will empower others to raise the bar in their practice and show our patients/members that we continuously strive to improve quality, engage, educate and empower others in our community.”

MercyCare Health Plans, along with several other departments across Mercy Health System, developed a diabetes disease case management program aimed to advance care processes, delay diabetes complications and save health care dollars. A significant part of the program educates members; it also focuses on physician outreach to provide compassionate, quality care to customers and patients.

The MercyCare Health Plans Diabetes Case Management Program was created ten years ago in coordination with the health care team to optimize health and prevent condition-related complications. With the additional support from a nurse case manager, patients gain education and self-care skills that improve overall health.

The MercyCare Health Plans’ diabetes case management program is available at no cost members who are living with diabetes and is designed to help them better understand and manage their condition.
