Dean & St. Mary’s Hospital to award $450,000 through new community health grant

Improving the health of our community takes more than just seeing the doctor. It requires the commitment and action of multiple organizations working together. That is why Dean & St. Mary’s is granting up to $450,000 to local non-profits interested in forging partnerships to benefit the overall health of Dane County.

“We know that making a true impact on the health of our community will take the help of many,” says St. Mary’s Hospital President Jon Rozenfeld. “It is our hope that the Helping Dane County to be Healthy grant program will be a meaningful step in creating lasting, effective partnerships in our ongoing journey to better health.”

Interested organizations must be a registered 501(c)(3) in Dane County. Local collaborations focused on improving health outcomes are highly encouraged to apply.

Grant recipients will partner with Dean & St. Mary’s Hospital on at least one of three recognized community health priorities. These health priorities were identified using community health data and feedback collected by the Healthy Dane Collaborative as part of a health needs assessment conducted every three years:

  • Mental Health, particularly focusing on reducing Dane County’s suicide rate
  • Chronic Disease, particularly hypertension, heart disease and type 2 diabetes
  • Maternal Child Health, which improves the lives and health of mothers and children in various ways

Over the three-year grant period, three organizations will receive up to $50,000 each year towards achieving their identified goals.

Proposals must be submitted no later than noon on Monday, December 7, 2015. More information is available on the St. Mary’s Hospital website.
