GHC-SCW Expands Network in Grant and Jefferson Counties

MADISON, WISCONSIN – Group Health Cooperative of South Central Wisconsin (GHC-SCW) announces partnerships with High Point Family Medicine in Lancaster and Fennimore as well as with Three Oaks Health in Johnson Creek.

GHC-SCW members can choose a Primary Care Provider (PCP) from one of two High Point Family Medicine clinic locations: High Point Family Medicine Lancaster Clinic and High Point Family Medicine Fennimore Clinic. GHC-SCW members in Grant County also have access to Urgent Care, Emergency Care and Specialty Care Services at Grant Regional Health Center in Lancaster.

“We are thrilled to be able to provide our Grant County members with more options for their primary care needs,” said Dr. Chris Kastman, GHC-SCW Chief Medical Officer. “Community-oriented partners like High Point Family Medicine allow our members improved access to quality care at a time when they need it the most.”

GHC-SCW members can now choose a Primary Care Provider (PCP) from Three Oaks Health. GHC-SCW members also have access to Urgent Care, Emergency Care and Specialty Care Services at Fort HealthCare in Jefferson County.

Insurance plans and benefits vary; GHC-SCW members are encouraged to call GHC-SCW Member Services at (608) 828-4853 to ensure their choice of Primary Care Provider.
